Nitrofurantoin is an antibiotic specifically designed to treat urinary tract infections, including bladder infections. Nitrofurantoin is unique because it concentrates directly in the urine rather than being absorbed throughout the body, making it particularly effective for urinary tract infections while minimizing systemic side effects. It's effective against the most common bacteria that cause UTIs, especially E. coli.
It's typically taken twice daily with food for five to seven days for acute bladder infections, or once daily at bedtime for long-term prevention in people who get frequent UTIs. Common side effects include nausea, loss of appetite, and darkening of the urine (which is harmless). Taking it with food or milk helps reduce stomach upset.
Nitrofurantoin shouldn't be used for kidney infections because it doesn't reach high enough concentrations in the kidneys or bloodstream to be effective for infections outside the bladder. It's also not recommended during late pregnancy or for people with significantly reduced kidney function.
Nitrofurantoin works by damaging bacterial DNA and interfering with how bacteria produce proteins, enzymes, and cell walls. Once inside bacterial cells in the urinary tract, nitrofurantoin is activated and creates reactive compounds that attack multiple bacterial processes simultaneously, making it difficult for bacteria to survive or develop resistance.
You may qualify for nitrofurantoin if you have an uncomplicated bladder infection or urinary tract infection, or if you need prevention of recurrent UTIs. It's commonly prescribed for straightforward bladder infections in otherwise healthy adults.
You'd generally be a good candidate if you have typical UTI symptoms like burning during urination, frequent urination, or cloudy urine, and no signs of kidney infection like high fever or severe back pain. Nitrofurantoin is also used for long-term prevention in people who experience frequent bladder infections.

If symptoms don't improve after two to three days or if they worsen, contact your healthcare provider as you may need a different antibiotic or further evaluation.
While there's no direct dangerous interaction between nitrofurantoin and alcohol, it's best to avoid or limit alcohol while taking this medication. Alcohol can increase the risk of side effects like nausea, upset stomach, and dizziness, which are already common with nitrofurantoin. Additionally, alcohol can irritate your bladder and slow down your body's healing process, potentially prolonging your UTI symptoms. For the best recovery, it's advisable to avoid alcohol until you've finished the antibiotic course and your symptoms have resolved.
While you can take nitrofurantoin on an empty stomach, it's best to take it with food or milk. Taking nitrofurantoin with food significantly reduces nausea and stomach upset, which are common side effects of this medication. Food also helps improve absorption and ensures the medication works more effectively. If you experience nausea even when taking it with food, try taking it with a meal rather than just a snack, or talk to your healthcare provider about strategies to minimize discomfort.
Nitrofurantoin can be used during most of pregnancy but is not recommended in the final month before delivery. It's generally considered safe during the first and second trimesters and early third trimester for treating bladder infections. However, it should be avoided during the last four weeks of pregnancy because it can cause potential complications in newborns, including hemolytic anemia in babies with certain enzyme deficiencies.
If you're pregnant or planning to become pregnant and have a UTI, your healthcare provider will weigh the benefits and risks and may choose nitrofurantoin or an alternative antibiotic based on your specific situation and how far along you are.
Nitrofurantoin doesn't typically cause drowsiness or sleepiness as a direct side effect. However, some people may feel tired or fatigued while taking it, which could be related to the underlying infection itself rather than the medication. If you experience significant drowsiness, dizziness, or feel unusually tired while taking nitrofurantoin, it's worth mentioning to your healthcare provider to ensure there isn't another cause or complication.
The most common side effects of nitrofurantoin include nausea, loss of appetite, upset stomach, and headache. Many people also notice that their urine becomes darker or brownish in color, which is harmless and goes away after stopping the medication. Some people experience diarrhea or gas.
Taking nitrofurantoin with food or milk can help reduce nausea and stomach-related side effects.
Less common but more serious side effects include lung problems (shortness of breath, persistent cough), liver problems (yellowing of skin or eyes, dark urine, severe fatigue), or numbness and tingling in the hands or feet, especially with long-term use.
