What is Saxenda®? How it Works, Benefits, Dosage, and More

Saxenda® is the brand name for liraglutide, a once-daily injectable prescription medication approved by the FDA for chronic weight management. It's manufactured by Novo Nordisk — the same company behind Ozempic® and Wegovy® — and was first approved for weight loss in adults in 2014, making it one of the longer-standing options in the prescription weight loss space.

Saxenda® is indicated for adults with obesity, or adults who are overweight and have at least one weight-related health condition such as high blood pressure, high cholesterol, or type 2 diabetes. It’s also approved for adolescents aged 12 to 17 with obesity — one of the few prescription weight loss medications with a pediatric indication.

How Does Saxenda® Work?

Saxenda® works by mimicking GLP-1, a hormone the body naturally produces after eating. GLP-1 signals to your brain that you're full, slows the rate at which your stomach empties, and helps regulate blood sugar. In people with obesity, this system is often dysregulated — hunger signals are stronger, fullness signals are weaker, and the body's natural appetite regulation isn't functioning the way it should.

By activating GLP-1 receptors in the brain regions that control appetite and caloric intake, Saxenda® helps reduce hunger and increase feelings of fullness after meals. Over time, this makes it easier to consistently eat less — and that sustained reduction in caloric intake is what drives weight loss.

It's worth noting that Saxenda® works alongside lifestyle changes, not instead of them. Like all GLP-1 medications, it's indicated for use in combination with a reduced-calorie diet and increased physical activity. Saxenda® Dosage and Administration

Saxenda® is administered as a once-daily subcutaneous injection — meaning it's injected just under the skin, not into a vein or muscle. It can be injected into the abdomen, upper thigh, or upper arm, and the injection site can be rotated between doses.

Dosing is gradual by design. Treatment starts at 0.6 mg per day for the first week, then increases in weekly increments until reaching the full therapeutic dose of 3 mg per day. 

This slow titration allows your body to adjust to the medication and helps minimize the GI side effects that are most common early in treatment. If side effects are difficult to tolerate at any point during titration, your provider may recommend staying at the current dose for an additional week before increasing.

The Saxenda® pen

Saxenda® is delivered via a prefilled, multi-dose injector pen that's designed to be straightforward to use at home. Each pen contains multiple doses and comes with a dial to set the correct dose before each injection. Needles are not included with the pen and need to be purchased separately — your pharmacist can advise on the appropriate needle size.

The pen should be stored in the refrigerator before first use. Once in use, it can be kept at room temperature for up to 30 days, away from direct heat and sunlight. It should never be frozen, and any pen that has been frozen should be discarded.

Saxenda® is injected once daily and can be taken at any time of day, with or without food — though taking it at the same time each day helps maintain consistency.

What are the Benefits of Saxenda®?

Saxenda®'s approval was supported by the SCALE clinical trial program — a large-scale Phase 3 development program that evaluated its safety and efficacy across multiple patient populations. 

Meaningful weight loss

Clinical trials have shown that adults taking Saxenda® at the full 3 mg dose lost an average of 8.4% of their body weight over 56 weeks, compared to 2.8% in the placebo group. Among participants who completed the full treatment period, average weight loss was even higher. For people who have struggled to achieve meaningful results through diet and exercise alone, that's a clinically significant outcome.

Cardiometabolic benefits

Weight loss with Saxenda® was accompanied by improvements in several markers of cardiovascular and metabolic health, including reductions in blood pressure, waist circumference, and cholesterol levels. For patients managing obesity alongside conditions like hypertension or dyslipidemia, those secondary benefits extend the value of treatment beyond the number on the scale.

Blood sugar regulation

Because liraglutide works on the same hormonal pathways involved in blood sugar regulation, Saxenda® can also help improve fasting glucose and insulin sensitivity — even in patients who don't have type 2 diabetes. In the SCALE trial, a meaningful proportion of participants with prediabetes at baseline returned to normal blood sugar levels during treatment.

Pediatric approval

Saxenda® is one of the few prescription weight loss medications approved for adolescents aged 12 to 17 with obesity. Clinical trials in this population showed significant reductions in BMI compared to placebo, making it a meaningful option in an age group where treatment choices are limited.

Potential Side Effects of Saxenda®

Like all GLP-1 medications, Saxenda® comes with a side effect profile that's worth understanding before starting treatment. The most commonly reported side effects are gastrointestinal — nausea, vomiting, diarrhea, constipation, and stomach pain are most frequent, particularly during the early titration phase when your body is still adjusting to the medication. For most patients, these symptoms improve as the dose increases gradually and the body adapts.

Saxenda® also carries a boxed warning for the potential risk of thyroid C-cell tumors, including thyroid cancer. It should not be used by anyone with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2. This warning is standard across the GLP-1 class.

Cost of Saxenda®

Saxenda® is a brand-name medication, and without insurance it can be costly. But, what you actually pay will depend on your insurance plan, pharmacy, and whether you qualify for any savings programs.

For insured patients, coverage varies widely. Some plans cover Saxenda® with a manageable copay; others require prior authorization or don't cover it at all. Novo Nordisk offers a savings card program that may reduce out-of-pocket costs for eligible commercially insured patients.

As with any GLP-1 medication, working with a provider who can navigate insurance coverage and prior authorizations on your behalf can make a meaningful difference in what you end up paying.

Key point: Is There a Generic Version of Saxenda®?

There is currently no FDA-approved generic version of Saxenda® available in the United States. Saxenda® is a biologic medication, meaning it's derived from a biological source rather than synthesized purely through chemical processes. Generic versions of biologics, called biosimilars, face a more complex and lengthy approval process than traditional small-molecule generics. While biosimilars for some biologics have made it to market, an FDA-approved biosimilar for liraglutide specifically for weight management has not yet been approved.

How Does Saxenda® Compare to Newer GLP-1 Options?

Saxenda® was a meaningful step forward when it was approved in 2014 — but GLP-1s have evolved significantly since then, and it's worth understanding where it sits relative to the options available today.

The most notable difference is efficacy. Newer GLP-1 medications have demonstrated substantially greater average weight loss in clinical trials than Saxenda®. 

  • Wegovy® (semaglutide), approved in 2021, produced average weight loss of around 15% of body weight in clinical trials. 

  • Zepbound® (tirzepatide), a dual GIP/GLP-1 agonist, has shown average weight loss of up to 22.5% — the highest ever recorded in a Phase 3 obesity trial. 

  • Foundayo® (orforglipron), the newest approval, delivers around 12.4% average weight loss in a once-daily oral format with no food or timing restrictions.

Saxenda® also requires a daily injection, compared to the once-weekly injection schedule of Wegovy® and Zepbound® — a practical consideration for patients who find frequent injections difficult to maintain long term.

That said, Saxenda® has a longer track record, a well-established safety profile, and may be the most appropriate option for certain patients based on their health history, age, or insurance coverage. The right GLP-1 medication is ultimately a clinical decision — one that should be made in conversation with a licensed provider who can evaluate your full picture.
